Use this form to submit an identity fraud claim

Before you lodge a claim, you will need to have the following documentation:

1. • Colour copies of ID. If providing a driver’s license you must include a copy of your Medicare card. Passports can be submitted without a Medicare card.
2. • A police report or event number, contact your local police for one.
3. • Any supporting documentation you've received from Vodafone showing the fraudulent connection or upgrade such as a welcome letter or recent invoice. If you don't have any of these, letters from debt collection agencies or emails regarding the account can be used.

To report the matter to the police;

• • Victoria Police accept complaints via your local police station. Please check the Victoria Police website for more info

You should advise the police you require a police event number as acknowledgment that you have reported the crime. If you're advised they cannot provide you with one please obtain the officers business card with their name and contact details and attach it to your submission.
